Who foots the bill on cross-examination transcripts in court reporting?

All out-of-court cross-examinations are not billed equally.

Court reporters tend to naturally assume that all cross-examinations are billed the same; that is, the examining party being responsible for the costs of all transcripts, including all copies.  However, exceptions to this practice do exist, particularly under Rule 39.02(4)(a) of the Rules of Civil Procedure.
